- Complete personal information and eligibility section.
- Sign and initial where indicated.
- Complete joint owner section, if applicable.
- Interested in a checking account? Complete the check order information section, select your choice of checking account, and initial.

The following must be submitted with your application:
- Valid copy of your driver’s license or photo ID
- Current address confirmation (ex. utility bill, financial statement, etc.)
- Check for $26 payable to "Visions Federal Credit Union"

Bring the application to one of our branch locations for processing or submit by mail, in which case, your signature must be notarized.

A temporary membership card, ATM/Debit card application, and required disclosures will be mailed upon receipt of application.
